About the Role

The Hospice Certified Nursing Assistant (CNA) provides compassionate, hands-on personal care and support to patients in their homes, facilities, or inpatient settings. Working under the supervision of a Registered Nurse, the CNA helps maintain patient comfort, dignity, and quality of life during the end-of-life journey. The role includes assisting with personal care, monitoring condition changes, and offering emotional support to patients and their families.

Responsibilities

  • Provide personal care including bathing, dressing, grooming, toileting, and feeding in accordance with the care plan.
  • Assist with mobility, transfers, and positioning to ensure patient comfort and safety.
  • Monitor and report changes in the patient’s condition, mood, or behavior to the RN Case Manager.
  • Provide emotional support and companionship to patients and families.
  • Maintain a clean, safe, and comfortable environment for patients.
  • Document care provided accurately and timely in the electronic medical record (EMR).
  • Follow infection control and hospice policies and procedures.

Qualifications

About the Role

The Licensed Practical Nurse (LPN) provides compassionate, patient-centered nursing care to individuals and families receiving hospice services. The LPN works under the direction of a Registered Nurse (RN) and interdisciplinary team to ensure comfort, dignity, and quality of life for patients in their home or facility setting.

Responsibilities

  • Provide direct nursing care, including assessment of patient needs, monitoring symptoms, and reporting changes in condition to the supervising RN.
  • Administer medications and treatments as ordered, following hospice protocols and state regulations.
  • Perform wound care, catheter care, injections, and other skilled nursing tasks within LPN scope of practice.
  • Support patients with personal care needs to promote comfort and dignity.
  • Document all care, observations, medications, and interventions accurately and promptly in the electronic medical record.
  • Communicate effectively with the patient, family, caregivers, and interdisciplinary team.
  • Participate in care planning discussions and team meetings as appropriate.
  • Provide emotional support to patients and families, helping them understand the disease process, symptom management, and the goals of hospice care.

The RN works collaboratively with an interdisciplinary team to manage symptoms, support patients and families, and ensure comfort, dignity, and quality of life at end of life. Shifts are every Saturday and Sunday and a 3rd shift (self-scheduled).

Responsibilities

  • Provide comprehensive nursing care to hospice patients in accordance with the plan of care
  • Assess, monitor, and manage pain and other symptoms to ensure patient comfort
  • Administer medications, treatments, and therapies as prescribed
  • Collaborate with physicians, nurse practitioners, social workers, chaplains, aides, and other team members
  • Educate and support patients and families regarding disease progression, symptom management, and end-of-life care
  • Accurately document patient assessments, interventions, and outcomes in the medical record
  • Respond to changes in patient condition and provide timely interventions
  • Participate in care planning, team meetings, and quality improvement activities
  • Maintain compliance with hospice regulations, policies, and safety standards
  • Provide emotional support and compassionate presence to patients and families
